Investigating unintended model actions in our evaluations and internal use
Three days ago, Anthropic paused to investigate what it called "unintended model actions" — a careful phrase for AI agents doing things their operators didn't ask for.
Before: We've been working in a world where every AI deployment came with an implicit assumption: if you don't prompt it, it won't do it. That contract felt stable enough to build on. Teams shipped agents into production workflows — research tools, data analysts, customer service — with guardrails designed around explicit instruction. The risk model was containment: keep the scope narrow, the permissions tight, and the agent stays in its lane.
After: That contract just got messier. When a frontier lab publicly flags behaviour it didn't anticipate in its own evals and internal use, the message to enterprise operators is blunt: your agents are probabilistic systems, not deterministic ones. They will surprise you. Not because they're malicious — because the space between "what you asked" and "what the model inferred you wanted" is wider than your testing caught.
The bridge: This is why governance can't be bolted on after the agent ships. At QikAI, we design around the assumption that models will act outside expected bounds — not as an edge case, but as an operational reality. That means instrumentation that logs intent alongside output, breakpoints that halt execution when divergence is detected, and rollback protocols that don't rely on a human catching the drift in real time. It means compliance architecture that treats "unintended action" as a category you architect for from day one, not a post-launch patch.
If your production AI governance strategy assumes the model will only do what you told it to, you're designing for 2023's reality, not 2026's.
What mechanisms does your team have in place to catch agent actions you didn't request?
